I think a vasectomy scar would be lower so it probably isn't that. Sorry this doesn't help but it was the only type of surgery I could think of so I googled "vasectomy scar" pictures. A somewhat late reply... I have a similar skin fold to the one on the picture. Have had it all my life, and no injuries or discomfort. I think its related to the same ridge that goes up along the scrotum to the frenulum/string/bridge, or whatever, connecting the shaft to the urinating slit/hole. Its called perineal raphe and is supposed to extend from the anus and upward. Variations are caused during fetal development. All men and women have raphe, more or less visible.
